Here is what to bring to your first visit: - Your completed Chiropractic Intake Form
- Driver's license
- Your completed Insurance Verification Form and Insurance card, if you are billing insurance
Your time with the doctor will be very focused and detailed.
Expect the following
- Verbal discussion of the paperwork you provided
- You may be asked to disrobe and wear an office gown and/or shorts. You may bring your own shorts if you prefer.
- A thorough physical examination. You will be asked to demonstrate various movements. The doctor will examine various structures of your body.
- A discussion of what the doctor found relevant and his assessment/diagnosis.
- A discussion of a treatment plan that will provide the most beneficial results.
- Time for you to address any questions you may have.
- Treatment will most likely consist of passive electrical modalities (Electrical Muscle Stimulation/Ultrasound), hands on myofascial release or massage, Hydrocollator heat packs or ice packs, and Chiropractic mobilizations or “adjustments”.
We recommend you call your insurance company and verify your Chiropractic benefits. Use the Insurance Verification form to help navigate your call to your insurance company. With completed insurance forms, a copy of your ID and insurance card we will submit claims to your insurance on your behalf. Your co-pay is due at the time of service. Dr. Lekas is in-network with BlueCross BlueShield.
- Car Accident
Our Chiropractors will bill for your auto accident injuries. Open an injury claim with your personal car insurance and we can submit claims on your behalf. Complete the Auto Injury Form and bring it with you to your appointment.
- Out Of Pocket
No insurance? No problem! We offer a 35% discount when you pay on the same day of your appointment. Or if you have a large deductible we can give you paperwork so you can personally submit to your insurance and still receive the discount by paying at the time of your visit.
